About Us

Braintree PCN Ltd, are a collaborative healthcare network dedicated to serving the local community.

We comprise three member practices: Blandford Medical Centre, Mount Chambers Surgery, and Blyths Meadow Surgery. We are also proud to host Silver End Surgery and Church Lane Surgery.

Our dedicated team includes Advanced Nurse Practitioners, Physician Assistants, Nursing Associates, Paramedics, First Contact Physiotherapists, Pharmacists, Pharmacy Technicians, Care Coordinators and Social Prescribers, all working together to provide comprehensive and accessible care for you and your family.


What is a Primary Care Network?

Primary Care Networks were introduced in 2019 as part of the NHS Long Term plan.  As the population grows and people live longer, often with more complex needs,  GP services have had to adapt to these changing needs.

GP practices are working together with community, mental health, social care, pharmacy, hospital and voluntary services in local areas in groups of practices known as Primary Care Networks.

Additional Roles Reimbursement Scheme (ARRS)

Primary Care Networks have been given funding to provide additional roles to create bespoke multi-disciplinary teams within their individual network. 

PCN’s assess the needs of their local population and, working with local community services, make support available to people where it is most needed.

Meet our Team

Lindsey Hunt – Business Manager

As Business Manager, Lindsey started her career in banking, but has since worked over 35 years within primary health care including Business Partner within a primary care partnership covering multiple sites. Subsequently taking on new challenges and transferring into primary care networks. She brings a wealth of experience in strategic, operational and project management along with enhanced business and leadership skills.

Lucy Morrish – Business Managers Assistant, Registered Nursing Associate

Lucy joined the Primary Care Network in 2023, having previously worked within general practice and secondary care hospitals for over 10 years. Lucy thrives on ensuring well-run operations exist within the Primary Care Network central hub allowing our organisation to succeed. In every position Lucy has held over the past 13 years, her prerogative has always been to ensure the patient experience is the priority.

Lucy Jessup – Physician Assistant and Transformation Lead

Lucy has been working for the PCN since 2024. She is a keen advocate for proactive health, the power of healthy lifestyle choices and their profound impact on disease prevention. When not working for the Primary Care Network, Lucy provides weekly healthcare teaching sessions within Anglia Ruskin University. Lucy oversees and runs the Clinical Research programmes for the PCN and uses this information to design new clinical pathways for patients.

Sonia McComb – Advanced Nurse Practitioner

Sonia is an Advanced Nurse Practitioner with the additional skill as a clinical nurse specialist in head and neck cancer. Sonia joined us in 2023 and helped design and run the PCN Acute Hub, an additional daily appointment route for the member practices. Sonia has worked within primary care for many years and brings extensive expertise and knowledge to her role.

Tracey Sell

Tracey Sell – Advanced Nurse Practitioner

Tracey is an Advanced Nurse Practitioner dedicated to women’s health and acute patient care. With nine years of experience as a matron in plastic surgery, Tracey brings a wealth of knowledge and a passion for providing outstanding patient care. Since joining us in 2024, she has working within our PCN acute hub providing additional daily appointments on behalf of the member practices. As an additional skill, Tracey has gained expertise in treating vaginal prolapse, ensuring every patient receives compassionate and specialised support.

Megha Upadhyay – First Contact Physiotherapist

Megha graduated in 2021 and completed her master’s degree in 2022. She has been working as a physiotherapist since her graduation and has developed a strong passion for delivering high-quality, evidence-based care that empowers patients to achieve optimal outcomes. Her expertise lies in Musculoskeletal physiotherapy. As a First Contact Physiotherapist (FCP), she provides expert assessment, diagnosis, and management of musculoskeletal conditions, ensuring patients receive timely and effective care.

Jay Shera – First Contact Physiotherapist

Jay started working for the Braintree PCN in 2025, with a background as a Musculoskeletal and Sports Physiotherapist. He brings seven years of experience in the field, having previously worked in rehabilitation clinics across the UK and with sports teams in India. As an FCP, Jay serves as the first point of contact for patients, utilising his clinical expertise to assess and determine the most appropriate course of action for patient care, including managing undifferentiated conditions.

Gemma Morgan – Care Coordinator

Meet Gemma, our dedicated Care Coordinator here at the Braintree PCN. Joining in 2023, Gemma has worked with people, their families and carers to improve their understanding of the patients’ condition and support them to develop and review personalised care and support plans to manage their needs and achieve better healthcare outcomes.

Emma Barfield – Social Prescriber

Emma is our dedicated Social Prescriber here at the Braintree PCN. Emma joined the organisation in 2022. Social prescribing can help to strengthen community resilience and personal resilience and reduces health inequalities by addressing the wider determinants of health, such as debt, poor housing and physical inactivity, by increasing people’s active involvement with their local communities. Within her role, Emma provides personalised support to individuals, their families and carers to take control of their well-being, live independently and improve their health outcomes.

Charlotte Barker – PCN Administrator & Network Coordinator

Charlotte is our Primary Care Network Coordinator and Cancer Care Coordinator. Charlotte joined the PCN in 2024 and is passionate about patient experience and collaborative working. As Cancer Care Coordinator, Charlotte works with the PCN team supporting them to implement and deliver the NHS England Early Cancer Diagnosis designated Enhanced Service.

Seamus Wishart – Paramedic

Seamus started working for the ambulance service as a student paramedic 11 years ago in the East of England Ambulance service Mid Essex division. Qualifying as a paramedic 8 years ago and continuing to work for East of England in his spare time. Seamus has worked for Braintree PCN for over 4 years and has just passed the First Contact Practitioner Paramedic course. He is passionate about improving care for the elderly and end of life patients that he sees.
